Death Claims

I will help you and your family complete all of the necessary forms for the death claims. To complete these, you will need:

  • Fee is $75, payable in cash or major credit cards. I DO NOT accept checks.
  • The death certificate.
  • Civil service annuitant claim number.
  • Marriage certificate if married.
  • Please keep in mind that additional information may be required.

If you are unable to make it to your appointment for any reason, please call with at least 2 hours notice so I can plan accordingly. Clients that do not show to their appointment, will be charged a $20 fee.

Medical Retirement

If you would like to set an appointment with me, please have the following information prepared:

  • The first medical appointment will take 1 hour then we will meet again after you gather more information from your Dr's Clinics and Hospitals, and we will finish up your package. My fee is $350.00, I accept cash and major credit cards. I DO NOT accept checks. Sorry for any inconvenience this may cause.
  • Your most current leave and earnings statement.
  • Your Bank Information for your Direct Deposit.
  • Your Spouse Name, SSN, Date of Birth, Your Marriage Date, Where you were married, You will need a copy of your marriage license to turn in with your retirement package.
  • Your DD 214 if you served in the military.
  • We will need to talk about Soc Security so if you have a statement bring it, if not, we will talk about Soc Sec how it applies to you and when you will need to apply for Medicare.
  • Your TSP balance.....go to tsp.gov, log in. then pull up your balance and print it out.
  • If you are WG/WS/WL, I will figure your high 3 from pay charts EXCEPT If you have had step raises, or pay changes because of promotion in the last 3 years Then you will need to follow GS instructions and bring your SF 50's with you.
  • If you are GS follow these directions:
  • Go to the AFPC secure website (the one where you look at EBIS for you benefits on base, click on the button that says Electronic OPF, after you sign in with your pin number, click on the LOAD VIEWER button at top of page. All of your SF 50's since you hired will come up I need to see all of them for the last five years. Click on the document name/employee name for each one and print each one and bring them with you to our meeting. You will have to click, open the form, hit print, hit the back button, and then click on the next date to print them all. This is a tedious process but a necessary one it is in your best interest to have a copy of all of them as this is your personnel file. I only need from 2008 to current.
  • If you pulled money out of the retirement fund because of a break in service, I will need to know how much and the date (month and year) of when the refund was received.
  • If you have paid back a military deposit, I need a copy of the receipt or a leave and earnings statement that shows the pay off, we can use this as a receipt for proof of payment.
  • I need a copy of any divorce decree you may have, and if it leaves your ex spouse any retirement benefits. If the divorce gives the ex spouse nothing, I don't need a copy.
  • I need to know former spouse names and birthdates and dates of divorce or death for your death claim paperwork.
  • I need to know your current office symbol and address, and your current series and grade and your Job Title.
  • If you would like your spouse or another family member to be present so they understand this process, please invite them along.
  • If you have had a Workers Comp claim where you have received a Scheduled Award, in the last two years..... I need Claim number and date of claim and how much your award was.
  • I need a list of all of the doctors you see as well as their address and phone number.
  • I need a list of all the clinics and hospitals and their addresses and phone numbers.
  • I need a list of all medications you are taking, what they are for and the side effects.
  • If you see Dr's or use the VA I need a copy of your VA files that pertain to the medical issue.

If there are documents you cannot find still come to your appointment and I will work around it.

Standard Retirement

If you would like to set an appointment with me, please have the following information prepared:

  • My fee is $250.00, I accept cash, VISA, or Mastercard. I DO NOT accept checks.
  • Your most current leave and earnings statement.
  • Your Bank Information for your Direct Deposit.
  • Your Spouse Name, SSN, Date of Birth, Your Marriage Date, Where you were married, You will need a copy of your marriage license to turn in with your retirement package.
  • Your DD 214 if you served in the military
  • Your latest Social Security Statement, if you have one dated within the last three years bring it.
  • Your TSP balance.....go to www.tsp.gov log in. then pull up your balance and print it out.
  • If you are WG/WS/WL, I will figure your high 3 from pay charts EXCEPT If you have had step raises, or pay changes because of promotion in the last 3 years Then you will need to follow GS instructions and bring your SF 50's with you.
